Most toilet problems — running, weak flush, a leak at the tank bolts — are repairable with parts that cost a fraction of a new fixture. We'll do that where it makes sense.
Replacement makes sense when the bowl or tank is cracked, when the fixture is old enough that it's using several times the water a modern one does, or when you're remodeling anyway.
Comfort height sits a couple of inches taller than standard, which most adults prefer and which matters a lot for anyone with knee or mobility issues.
One-piece units are easier to clean with no seam between tank and bowl, and cost more. Two-piece is the standard and perfectly good.
Modern low-flow toilets from good manufacturers flush better than the early ones did. If you replaced a toilet in the 90s and hated it, the technology has moved on considerably.
